Welcome to the SafetyPlusWeb™ CQ Start-Up Page. As part of Shawmut Design & Construction’s General Project Requirements, contractors performing work must participate in the SafetyPlusWeb™ CQ & Site Tracker program in order to qualify their companies and workers.

Step 1 – Registration and Set-Up Process
- Complete Registration Form on this page to send your basic information to Safety Plus. After you send your basic information using the Registration From, you will be taken to the secure Order Form. Place your order.
- A Safety Plus Team Member will create your account and email you your login credentials and tutorial videos for SafetyPlusWeb™ CQ and Site Tracker.

SAFETYPLUSWEB™ CQ IMPLEMENTATION PROCESS
Step 2: Questionnaire – Company Qualification
Upon receiving an email with your SafetyPlusWeb™ CQ credentials, proceed to CQ.SafetyPlusWeb.com and login to your profile. You will need to complete all questions along the left panel of your Contractor Dashboard. Incomplete sections are indicated by numbers and red dots. As you complete each section, select “Refresh Red Dots” on the bottom of the left panel in order to update your progress. As you progress through the questionnaire, you will need certain information and documentation. This includes, but is not limited to:
- OSHA 300A logs (previous 3 years); Must be completed and signed. (You can optionally send these logs directly to [email protected] and our team will upload the logs and answer the appropriate questions for you)
- EMR rates (previous 3 years); Must be on official insurance letterhead. (You can optionally send these rates directly to [email protected] and our team will upload the logs and answer the appropriate questions for you)
- Your company’s safety manual

Status and Follow Up
Once you have completed the SafetyPlusWeb™ CQ Questionnaire, allow 5 business days for a Safety Plus team member to review your profile. When review is complete, you will receive a Status Update email indicating that your company has been marked as Complete, Conditional, or Failed. If you receive a Complete or Conditional status as a company, your company is now considered qualified.
Step 3: SafetyPlusWeb™ – Employee Qualification
In the same email that you received login credentials for SafetyPlusWeb™ CQ, you also received a link to a SafetyPlusWeb™ employee registration page. Your employees will use this link to register for the site, take orientation training, and provide proof of other required trainings and or be able to take trainings online (online training is included and does not have any additional charges).
